Community Support Services

SJBHR also has a community supported housing program for individuals who have serious physical health problems in addition to a diagnosis of mental illness. Varied housing options, different levels of support, and flexible services enable the program to meet the unique needs of each individual and to support them on their journey to wellness and recovery.

  • Medication monitoring
  • Assistance with academic and vocational services
  • Increasing social and recreational opportunities
  • Money management assistance

Admissions Criteria:

  • Diagnosis of a severe and persistent mental illness
  • 18 years of age or older
  • History of extended or repeated psychiatric hospitalizations
  • Co-existing mental illness and substance abuse
  • Demonstrates the capacity to live 1 hour or more but less than 4 hours of daily support and/or supervision to maintain stability.

Referral Process:

Residential:

State and County Hospitals- Please contact the Director of Residential Services via email and provide the consumer’s name and current location. All other needed history will be obtained at the time of the interview

Community Support Services:

Please fax referral information including copies of psychiatric evaluation, assessment, medical history, lab work, mantoux record, drug profile, insurance information, financial information and legal history to the Director of Residential Services.
